    Sup fa/tg/uys. I have come to tell you something awesome. My BBEG is an illithid. Big whoop, you say. However, this is an illithid with levels in Fleshwarper. He's a grafting fiend; after he saw how effective grafts were on thralls, he figured they'd be even more effective on a superior race, i.e. his own. He's gone a tad bit overboard.
    He's an illithid with a replacement heart, tiny hooks on the end of his mouth tentacles, a telescoping mouth that can paralyze those it bites to allow the tentacles to grab their head and begin brain-munching (an alteration of Mohrg Tongue), a venom-filled scorpion tail, two replacement stilt-like legs ripped off osyluths, a eye resembling molten gold that is a Charm Monster gaze, and bug-like Antennae on his head as well as thousands of little hairs on his body, which give him tremorsense. His familiar is a semigelatinous, scale-covered, halfling-sized weasel with insect wings, a mass of tentacles around its mouth, and razor-sharp teeth.

    Keep in mind that its not just able to graft onto itself: It'll give grafts to its favorite minions. Hell, even its lackeys if they're cheap enough. Imagine a swarm of pseudonatural hobgoblins with arms bigger than they are and mouths open farther than their skulls should allow, wailing in agony, begging to be killed to be released from the pain that is caused by their bodies forcibly rejecting the shabby, half-assed grafts the illithid gives to the cannon fodder.

    With a Hat of Disguise, the Illithid could even set up shop selling its grafts. Nondisfiguring, only partially invasive grafts for the common folk at ridiculously low prices, easily at a loss. What farmer wouldn't want bulkier muscles? A stronger heart? Legs that wouldn't tire?
    Of course, theres a catch. The Illithid has just gotten done with a spell that controls those with grafts, and Illithid grafts already impose penalties on will saves. After a while of selling, the Illithid proceeds to dominate the entire town, gets all its money back from them, and has dozens (hundreds?) of new subjects to experiment on, and to use as fodder versus those pesky investigating PCs.

    For BBEG this is manipulative and evil...AND AWESOME!!! The only way it could be better if your party interpreted you as one of those "brutal" DMs, the ones that players are always fearful of combat, but the reason they don't get up an leave is because the storytelling made the pain worth it.

    I like the idea that the Illithid could set up shop in a small, rural town, using the CR 1/4 and 1/2 monstrous races in the surrounding wilds for the brunt of his experiments while reserving the humans as test subjects for the occasional largely-finalized "covert" modification. He plans to infiltrate human society using agents with enhanced phsical capabilities and a set of parasitic and symbiotic organisms that can reproduce and infest others to create new "converts". For an added bonus, have his innocuous home base be the exact same low-level region that the PCs began in- his infestation was already halfway complete when they left to bigger and better things.

    You can't approach NPCs from the same perspective as PCs. Sure, a class might be a waste on a PC, but that's because you have limited oomph. NPCs don't. They only have the limit on their levels of common sense.

    Also, clearly only his superior illithid brain is capable of unlocking the full potential of fleshgrafting.

    I know its mostly for the flavor and all, it just seems a bit unnecessary to to add a few class levels simply for the title the class gives.
    The NAME of a certain class doesn't necessarily have to do with flavor at all, especially not in a BBEG.

    That said, I suggest the OP to grab the leadership feat (Not that a illithid would need it in most cases) and make a decent Fleshwarper out of it rounding it out with something arcane.

    The BBEG can still be seen as a body augmenter of any kind, and his assistant simply helps him perform the alterations on his body.

    He can still "do it himself", you don't need to have fleshwarper class levels in order to have some knowledge about the subject. Heck, he can be the master of flesh warping without a single level in it.
    You just need a bit of creativity and a back story.

    Again, the character in question is a BBEG if the DM wants him to be a fleshwarper then so be it.
    But adding class levels is not the only way to get grafts, and for a BBEG which sooner or later will be fighting the PC's one way or another, levels could be better spent making the final encounter(s) more interesting.

    And honestly, what's the difference anyway? The PC's will in most cases not know the BBEG's stats anyway.
    He will get the same grafts with the same "flavor" but saving a heck load of levels taking the Leadership feat instead.

    All in all:
    "Pure" Class Levels is usually not a good basis for
    flavor.
    A BBEG can always work around it some way or another, in order to avoid what can (sometimes) "useless" levels.
