    My advice is to watch Silence of the Lambs and study Hannibal Lector very closely. Hannibal is probably the coolest serial killer in existence.

    Hrm. Are we going habitual tick or obsessive compulsive?

    The former could be like pressing his forearm against the flat of a blade, a habitual response developed to silence a blade being drawn from it's scabbard. This could have several visual cues.

    For the latter, it might be hard to do without seeming forced. Subtle is better than obvious, and the ability for the viewer to eventually learn WHY it happens and go back and look at that odd behavior is kind of fufilling.

    If said villain is generally going to be encountering the party at a distance, make them severely avoidant of interpersonal physical contact. Really, play them as an abuse victim or a PTSD sufferer, if you feel able to do it tastefully. The impact of these things on a person's behaviour are not only highly visible in appropriate circumstances, but pity-inspiring as well. A word of warning, though, do your fucking research so you avoid throwing out another ball of trite cliche.

    Just make a BBEG that's a liar.

    Right though his teeth, every time. He lies about his accomplishments, he uses illusion spells to make himself look tougher and have battle scars for fights he's never been in. He wears a mask across half his face, claiming he earned it from a belor fight, stuff like that.

    He'd be an enchantment wizard, using the spells to boost his ego. He'd never kill the characters because he wants someone to be after him, he wants to feel important.

    Have him bite his nails. Not just when he's nervous or bored, but all the time.

    Have him sit on chairs strangely, like one leg always thrown over one of the arms, or even just have him perch on the seat like some sort of bird.

    As part of the realising 'holy shit I'm kind of heading out into insane territory here', have him order his lieutenant to stop him if he tries doing whatever his compulsive thing is, like eating the fingers of people he's killed or something insane like that.

    For bonus points, have him fully aware of the fact that he's probably going completely insane and his most trusted lieutenant should be his sort of 'moral compass', stopping him from doing things that are a little TOO insane. When the PCs inevitably kill this lieutenant, the villain goes COMPLETELY off the deep end.

    An example of this is Dio from Jojo's Bizarre Adventure: His 'evil plot' in season 3 is actually extremely admirable: He wants to create a perfect world for all Stand users, where they can live freely and use their powers as a sort of system of ruling nobles/police force. However, he's well aware of the fact that he loves to kill people, hates people who foil hi 'brilliant' plots and has a HUGE ego, so he actually asks one of his followers (one who isn't being mind controlled and is still absolutely fanatically loyal) to keep an eye on him and stop him if he looks like doing anything stupid, in case he 'gets distracted'.

    Come to think of it, Dio is a pretty fucking amazing villain.

    Hydrophobia. Not OH MY GOD THEY THREW WATER ON ME NOOOOOOOOOOOOOO hydrophobia, but make the guy absolutely goddamn terrified of deep water. If the heroes make their escape (or attack) by ship, he won't even scry them to find out their numbers, in case something goes wrong and he ends up out there in the deep dark sea.

    Alternately, have him absolutely refuse to kill women or children (to the extent of trying to knock them out or just plain running away if one of the PCs is a woman or, god forbid, a child), and actively hunt down those who do within his 'patch'. If by some fluke he does accidentally kill a child, have him simply stand there staring in disbelief until he's quietly led away.

    For a more serious route, I've always thought about taking the standard "organized" serial killer, and suggesting that he uses enhancement magic to fulfill his fantasies rather than murder. He's an amazing mage, extremely well studied and practiced, but he just uses his powers for personal gain, wandering around abusing the shit out of them for personal gain and advancement. Eventually, he can't be satisified with this, and decides to becomes someone who everyone will remember.

The BBEG actually really detests killing people/etc, but hes so devoted to his gramps that if his big plans called for it he would. He'd be awkwardly bad at villainy ("I'll knock you all down! ...wait, that sucked, let me try again...") for a while, but eventually he'd find The Amulet Of Great Super Powerfulness, and then he'd throw together raids that would make his grandfather proud...

    Maybe he could be obsessive about something. So much so that he'll go far out of his way to get it, or interact with it. Everything else is not only unimportant, but also irrelevant. He'd throw his own mother away for it. He'd spit on god just to see it. He'd piss on his own grave just to smell. Thanks to this obsessiveness he's amassed huge resources and connections, and made a few promises he never intended to keep.     I had an idea which was generally well-received around here about a pure Lawful Evil villain who had managed to sleep her way to the top and then charm or dispose of everyone who opposed her.

    She was the head of a school of sorceresses based on ritual mutilation, who gained their powers from harming themselves according to a series of rituals - particular patterns of scars, using parts of their own bodies for rituals with an organic element and so on.

    Her plan was to stage a coup, starting by eliminating the brightest and best children of the nobility by either killing them in "accidents" or attracting them into her school of magic and turning them to her side.

    No word of a lie, I actually did this in a campaign as a side-session since not all of my usual bunch could show up that week. The players were responding to a request for help in a Dark Lord's Bakery. The final baker-boss had a MASSIVE muffin-chef's hat. He broke off and forcefed some of the players, who then developed crippling addictions to said muffins due to some comically low fort saves (I was just going to make them calm and sleepy, but DAMN, 1 ones in a row HAS to have penalties)

    So, of course, they killed him and now have to stop off at the bakery whenever their precious foodstuffs level dropped low. They even had a bag of holding filled with the damned things.

    Even my throwaway sessions seem to have huge impacts on the rest of the game.

    Its more an idea for a campaign but meh:

    The kingdom faces invasion by hordes of the possessed and a madman roams the streets of the capital, killing important leaders and ransacking wizardry research labs searching for a way to cure the madness.

    Actually: Hes a raving lunatic because of the backlash from a ritual to summon a sinister unworldly supernatural entity, but all of his actions are somehow tied to trying to banish it back to where it came. The party is unaware of the second part and is serving the ruler of the kingdom that is using the power of the summoned entity to hold back the tide of an invasion from a neighboring country that has been possessed by another powerful demon. The truth is the demons are one in the same and its method of gaining control over people is by creating problems then tempting people with the power to resolve them.

    The madmans quirk could be leaving pleas for forgiveness and help to god at the scenes of his antics.
