I'm going to second that the first idea is a great on on the principle of roleplaying and such. It'll handle thje in-character side of things. And it's an amusing way to gainsay his OOC bitching about the game, by treating it as IC. This is not at all a bad way to handle this sort of thing, as putting some in-game consequences to certain kinds of player behavior can work.

beyond that, I do suggest at least some talking to him OOC, as the issue is mostly an out-of-character one in which this guy is bitching for the sake of bitching. It'll reinforce any in-game yelling at him for his shortcomings and that if he wants to stay alive, he would do well to get a better sense of tactics and such. Basically, say it OOC too so he knows you're upset with him in general. And you can add stuff like asking him why he's playing a game he so vocally dislikes. My group kind of kicked a guy out from any games with certain systems because he wouldn't stop griping about said system during play.

I feel your pain. We got a guy that brings his kid with him to the encounters and they both suck. The dude plays a sorceror and at least stays in the back, he just doesn't get team work at all. The kid plays a fighter or paladin I forget which because he apparently took a level of corpse. As the healer I tried to keep him on his feet, but quickly decided to keep the more valuable players alive instead.

If you have the compasssion, I would suggest introducing him to things that might increase the usefulness of his healing surges. There are potions that offer regen for bloodied characters that last for the encounter (no books here, but its in the Adv Vault) so for five minutes he can keep healing with the one surge. We have these for the guys who end an encounter below 1/4hp, since a 50(ish) gp potion will get them to half. My Warden had one at all times, in a wineskin so it would never shatter. SO, you could offer game mechanics advice which would make his character more effective thus helping him out which might make him a better player and a better gamer and have more fun, thereby he may actually stop sucking the proverbial cock.
