Could be the consorts, courtesans and flesh-traders who are deemed unfit for further service. A vacationing merchant takes a knife to the working girl he was chaperoning for the night, and she's given the choice to serve silently in the PDF or be left to bleed out. The madame of a mid-level bureaucrat gets replaced, and is told to join the guard or find her own way on the streets. They might also receive more cosmetic adjustments, as well as the heavy physical augmentation, to resemble some founding member/s of the pleasure world as a display of its vanity.

Like actual peasants, yes they have the duty of protecting them, but that doesnt mean they have to treat them well. On the other hand, they treat their allies better, they still keep a lot of secrets from them, but it's better than nothing. This long and pervasive history between the chapter and their personal tombs may be to allow the chapters priests/chaplains or some such the ability to control them via suggestion and compulsion back to their tombs after battle.
Idk though, if people are ok with idea of them being hypno indoctrinated that way and it working somewhat even in the throes of the black rage (though it would be latest somewhat explained by them being indoctrinated, and physically remade as exanguinators do >>82580795)

I imagine due to their reclusiveness they view as useful. More so than other PDFs or Guard Regiments that they've worked with. They have a low opinion of mortals and see them as children that bicker amongst themselves. The PDF they just tolerate the most.

I’d imagine gene seed is psychically active material, not to mention the blood angels all being latent pykers, I was planning on the tombs being lined with the blood of the death company, the amniotic fluid in the exsanguinators/tombs also being blood from the death company…
Pretty much the most valuable resource I was going to allude to for this chapter would be their own blood, especially the blood of the oldest marines/surviving death company.
A lot of their spirit journeys / hypno music in dreams while entombed all being for the purpose of actually drawing out the life of a marine once he’s consumed by the black rage, completely opposed to the doctrines of the other blood Angel chapters in that regard.
They would imagine there to be a hidden epiphany hidden in the blood of those embraced by the black rage, and lying at the end of the black rage in death, and behind the black rage of those who have mastered.
They aren’t entirely wrong, at least in the regard that overcoming the black rage seems to have turned mephiston into a Super Blood Angel 4.
It’s just not really working in the way they would hope yet, even if they aren’t at all discouraged and are instead only encouraged by each advance in their pursuits, real or imagined.

Achieve perfection “OF THE BLUD” no less, or least mastering and understanding their dead primarchs messages he send through it, using “gene seed” memory and voodoo like divination using their own bodies as the fetish, and red thirst filled dreams as the psyche/body killing “substance” they would imbibe to enter their spirit walking journeys from their tomb. Was kind of thinking, like how the grey knights form a gestalt to hunt demons, or the emperor is a gestalt entity made on himself, maybe in their dreams, they form a gestalt creature who is, again, using their blood as a portal/medium to explore totally not! The warp for purposes of finding the source of black rage and the message/meaning it holds, of which they believe is their Primarichs doing, so in effect they are looking for sanguinius’ soul in the warp, or at least wherever the “physical” location in the immaterium (a bit silly, I know) that he needs them to discover in the black rage, and the waking messages it provides them they are blind and deaf too.
Essentially, these guys have givin up on reality outside of their duty as marines: war, and are spending their entire existence as a chapter outside delving into the perceived “final gift” of sanguinius to his sons, & any potential the blood angels at large are obviously squandering by not letting it embrace them and mastering it properly as, they believe, he would have wanted them too.
