    Use Flensing (CM), then enchant an amulet to continually apply the Internal Fire (CM) spell to the wearer. As a final touch, give him a circlet that has an insane regen rate enchanted into it.

    Your AHNM AIMS has now been upgraded to include flaying the man while he can only watch, giving him the worse case of Heart Burn (or Organ Burn, as the case may be) in the multi-verse, and he constantly regenerates the damage, so he does not die.

    To begin with, use the Transfix spell from Complete Mage. It'll paralyze them for the duration, and they'll be aware of it. The Flesh to Stone + Shape Stone mutilation is a nice touch, but lets crank it up a notch.

    Turn it into a Golem while it is Stone. You now have a Stone Golem. Use Stone to Flesh. You now have a Flesh Golem.

    Use Awaken Construct from Spell Compendium to make your Flesh Golem sentient.

    Making him a Flesh Golem wouldn't work out all that well. It is implied that they feel no pain (and that the Frenzy ability is just their metabolism pumping them up to prolong their usefulness, not an actual, emotional reaction to pain).

    This thread has been cranked up to 11 on the Nightmare Fuel level.

    How about we break the knob?

    Add the Creeping Cold spell from Spell Compendium to the mix. Burnt alive, bodily organs/fluids being pushed out through the skin, and now fluids on the outside of the skin/inside the skin freeze solid and break, giving you frostbite and festering, frozen, flaming, sores.

    Then, to make matters worse, add on top of that a Dire Hunger spell, which basically turns them into a ravenous eating machine that feels terrible pain if it isn't eating something. It still lets them keep their sentience though. So now this man is abominably hungry to the point where if he could, he would quite willingly eat his own torso to fight the hunger. Except he is still held in place, due to the Stasis spell placed on him.

    The true torture comes from being aware of the suffering.
    Like a cursed ring that once put on, casts time stop continuously, and can't be removed. You age your entire life before anyone else can do anything.

    Attempting to contribute, but there shouldn't there also be something like Programmed Amnesia (CA)? Wiping their happiest memories away, amplifying their worst memories to be even worse, make them lose track of time and give them painful memories of infinite suffering, whether or not they happened. The other spells crush them physically, sure, but their mind needs a good flensing as well, agreed?

    Psions are actually better at this.

    Silopsism. Put them in their own head, in a dreamworld of your devising. They lie there comatose, until such time as you release them....or they die. And since it takes minimal effort to feed them, you take their loved one, explain the situation....and then lay an attraction effect with a permanent duration on them , making them attracted to the idea of their loved one remaining alive with them.

    After turning them both into Elan so they live FOREVER.

    An excellent point, and I applaud that initiative. I think we could also add the Maddening Whispers spell and the Nightmare Lullaby spell on top of that.

    Create memories that start off happy and eventually degrade into horrifying fates. Use the Maddening Whispers to be a low background hum at the beginning, then becoming a deafening cacophony once the Nightmare Lullaby/Programmed Amnesia memory reaches its peak.

    Then, the nightmare seemingly ends, and a different memory boots up, starting happy. And it will end the same way. Ad infinitum.

    He is vaguely aware of his condition at the start of the new memories, and becomes increasingly aware as the nightmares progress.

    No, what we must do, is bring the man back from imprisonment every other month.

    First, we obliterate the minds of his loved ones, crafting our own reality for them. Make them be our willing slaves of their own free will (after the mind-rape, that is).

    Then, we bring the poor bastard back in a room with his loved ones gathered inside, and release his bonds.

    Once he's eaten them, bind him again, and imprison him again.

    That's a sign you're not doing it right. If you've done it properly, his mind has been so warped by the torture that he can't even form a coherent thought.

    What I'd do is, every few years or so I set it up so that he can escape from his torment, then when he corners me I exclaim "how did you stay sane?!" When he responds, I incapacitate him using a failsafe I already installed inside him. He is paralyzed, but not hurt. The fear would be the worst part; one thing making him unable to enjoy his brief reprieve. I then modify his torture so that the thoughts that kept him sane are leached out of him. When it becomes impossible to get him to gather the willpower to escape, it means I've won. He is now such a pathetic, broken creature that he cannot fathom the concept of hope, or believe that there is anything beyond suffering, thinking that his life before constant torture was just a strange dream he had.

    you forgot something, very important. At an irrigular (and infrequent) intervals you need to let him experience unadultered joy and happyness, for lets say 15 minutes. Remember suffering is all fine and dandy, but without a fram of reference, its just becomes normal. Allowing those breif (and ever more infrequent) times of happyness help keeps things in perspective. The persepective in this case of course being horrid tortures.

    That's a fun thought. I don't know about the game mechanics of it, but it might be fun to have the person "wake up from a nightmare", with some memory manipulation and the like. Just wake up in a surrounding what seems like reality for one day, with the memory of the long perioids of torture a little fogged but still vivid and horrifying for a dream. Then watch him squirm and struggle against falling asleep again. And when he finally falls asleep, start the whole process again.

    For extra points, the villain should propably pretend, with help from memory manipulation spells, to be a family member or a close friend who'd be present in that brief "waking world" perioid.

I figure if you alternate the pattern every so offten, you could achive a state of all consuming agony that they could never get used to. That would last about as long as you could mechanicaly keep them alive. I thought about this in case we ever catch Bin Laden. :)
